Another Vigil Service Bulletin

 APF Service Bulletin No APF 091110 – Issued 10th November 2009

 Background

An incident occurred on the 19th of September 2009 in Belgium, in which a cutter body separated from its base on a Vigil AAD (see attached Incident Report. The original was originally received in French. I have translated and been advised that this translation is acceptable)

 Advanced Aerospace Designs (AAD) have completed their investigation of the Vigil unit and concluded that the cutter was part of a batch produced in December 2007. They have released a Service Bulletin that requires the recall and replacement of the affected cutters. Their SB requires the cutters to be replaced at the next reserve repack cycle.

 Though the Vigil PSB-5 has an issue date of October 10th 2009, this bulletin was not made available to the wider skydiving community until early November.

 The APF has released mandatory APF SB 091110 on the 10th of November, which takes precedence over the Vigil PSB-5. It includes the following APF Policy regarding the Vigil PSB-5

 APF POLICY: These cutters are to be replaced PRIOR TO NEXT USE for each affected unit. The cutter date of manufacture is printed under the part number 9582006 in the format 12-07-2 (mm/yy/batch). A hologram number is found on the cutter with a corresponding hologram on the main box. If the cutter has been upgraded from type 1 or 2 to 3 after December 2007 you will need to check the date of manufacture or hologram number against the list issued as part of AAD PSB 5 – the reserve container will need to be opened to inspect. Owners are to contact AAD VIGIL or dealer for replacement cutter.

 Attached to this email is the APF SB 091110, which includes the Vigil PSB-5 plus list of affected cutters.
